How Does Insurance Work for Therapy? Everything You Need to Know (But Probably Do Not Want To)

Let us be upfront with you right from the start.

This blog is about insurance. Specifically, how health insurance applies to therapy. We are talking premiums, deductibles, copays, and coinsurance. The stuff that makes most people's eyes glaze over within thirty seconds.

We completely understand if that is you. No offense taken.

If you are the kind of person who would rather just hand this over to someone and say "tell me what I owe," that is exactly what we do at Bluebird Therapy Center. You fill out a form, we run your insurance, and we come back to you with clear, specific numbers. Simple as that. In the meantime, here are two other blogs you might find a lot more interesting while we handle the insurance side for you:

We will be here when you are ready.

For everyone still with us... let us get into it.

The Four Terms You Need to Know

Before you can understand what you will actually pay for therapy, you need to understand four specific terms. Once you know these, the whole system makes a lot more sense.

Premium

Your premium is the amount you pay every month just to have insurance. You pay this whether you use your insurance or not. Think of it like a subscription fee for access to coverage. Your premium does not change based on how many therapy sessions you attend. It is a fixed monthly cost.

Deductible

Your deductible is the amount you have to pay out of pocket for healthcare services before your insurance starts contributing. For example, if your deductible is $1,000, you will pay the full cost of your therapy sessions until your total out-of-pocket spending reaches that $1,000 mark. After that, your insurance begins sharing the cost.

Important note: not all plans require you to meet your deductible before covering mental health services. Some plans cover therapy copays regardless of where you stand on your deductible. This is one of the key things Bluebird checks for you when we run your benefits.

Copay

A copay is a fixed flat fee you pay per session. If your plan has a $15 therapy copay, you pay $15 per session and your insurance covers the rest. Copays are usually straightforward and predictable, which most people prefer.

Coinsurance

Coinsurance is a percentage split between you and your insurance company after your deductible has been met. If your coinsurance is 20 percent, you pay 20 percent of the session cost and your insurance covers the remaining 80 percent. This is different from a copay because the amount you owe can vary depending on what the session is billed at.

How Different Insurance Types Compare

Here is a simple breakdown of how these four terms apply across common types of insurance:

Medicaid is the most straightforward. If you qualify for Medicaid in New Jersey, your mental health coverage is typically provided at no direct cost to you. No premium, no deductible, no copay, no coinsurance.

Medicare involves a modest deductible and a small coinsurance percentage for outpatient mental health services after that deductible is met. Most people with Medicare find their out-of-pocket costs for therapy to be manageable.

Commercial insurance plans are where it gets complicated. A United Healthcare plan selected by one person and an Aetna plan selected by another person can look completely different even though they are both commercially insured. The deductible might be $500 or it might be $3,000. The copay might be $20 or it might be $60. Coinsurance might kick in or might not. It depends on the specific plan you chose during enrollment.

"It Depends on Your Insurance"

We have all been there. You call a doctor's office and ask what you will owe. The person on the phone says those five words that are somehow both completely accurate and completely infuriating:

"It depends on your insurance."

Here is the thing. That answer is not the fault of the person saying it. It is not the fault of the doctor or the front desk staff. It is the honest reality of a system where two people with the same insurance company can have completely different coverage based on which plan they selected when they enrolled.

The frustration is very valid. The system is that complicated. (If we get enough interest we can post a blog about how the system got so bad) And blaming the person relaying the information has never made anyone's copay lower.

What does help is getting specific information about your specific plan, which is exactly what Bluebird does for every new client before their first session.

The Bottom Line

The insurance system is complicated by design and almost nobody enjoys navigating it. You are not supposed to find it intuitive. It is not.

What matters is that you do not let confusion about costs become the reason you put off getting support for your mental health. That is too high a price to pay for avoiding a phone call.

At Bluebird Therapy Center in New Jersey, we make this part easy. Virtual sessions are available across all of New Jersey. Most major insurance plans are accepted. And we do the legwork on the insurance side so you do not have to.

The hard part should be asking for help, not figuring out what it costs.
